For Inpatient Nurses
Effective Sunday, Nov. 11, interventions will appear in the Braden Scale and Wounds assessment based on cascading subscores, as well as the total score. You should document the cascading interventions within each section as they appear.
For patients with no wound risk, you won’t see any changes to the Braden Scale documentation.
Row-specific interventions will replace the severe, high and medium risk interventions previously based on the Braden Scale total score. New interventions also will appear in the Wound section based on the documented wound type (pressure ulcer, other ulcer or wound).
