When a patient has been admitted for four hours and the PTA (prior to admission) medication review is complete, but the PTA medication reconciliation isn’t, a new BPA will display alerting you that PTA reconciliation is needed/incomplete. Go into the Admission Medication Reconciliation process, and act on every home medication to ensure each one is accurately continued, discontinued or further modified.
