As part of an HM initiative around social determinants of health impacting our patient population, we’re making it easier for you to access the Social Determinants of Health (SDOH) Wheel in Epic. The SDOH Wheel helps you identify and document on patients’ social, behavioral and economic factors impacting them outside the health care setting. It includes 12 domains with screening questions, designed to uncover factors like housing status, food insecurities, social support and more.
You can now access the SDOH Wheel link on all inpatient clinical Storyboards. This will help you to easily identify, assess and address your patients’ SDOH and screen them for any health-related social needs.
