This week, Houston Methodist launched our first Clinical Pathway with an HMH colorectal inpatient pilot. Clinical Pathways is a streamlined documentation and decision-making initiative to improve patient outcomes tracking during hospital stays. To date, two patients are currently on a pathway.
“Congratulations to the team for all your hard work in making this Clinical Pathways a reality for our patients,” said Pat Harrison, vice president for system quality and patient safety. “Having these standardized protocols is a significant step forward in improving our patient care.”
Clinical Pathways provides structured, care plans for patients with specific clinical problems, helping improve continuity and care coordination across the patient experience. It will help guide evidence-based care, reducing care variation, length of stay, readmissions and costs.
