HM Funeral Home Release Form Updated in Epic
Effective Now
To meet legal, quality and patient safety standards, the HM Funeral Home Release form in Epic has been updated.
Changes to the form include:

Heading name change.

Addition of facility name, name of person signing the death certificate, body disposition documentation, name, address, phone and fax numbers of the funeral home or transport service, location where the body is picked up from and new definitions for added clarity.

Legal language updated to comply with requirements.
