Discharge Order Option: Add Patient to Communication/Education Pathway for Sepsis
When a patient meets Heart Failure criteria, you currently have the option in the Discharge order to send educational communications to the patient. Starting Monday, Jan. 30, this option will also be available for patients meeting Sepsis criteria.
When you select Yes to Add patient to Communication/Education Pathway for Heart Failure and/or Sepsis:

New language will be included on the AVS cover letter, indicating that the patient will receive educational communications about their diagnosis.

Your patient will receive these communications via their cell or email, if one was provided.
