To ensure quality measure compliance, make sure you are correctly documenting your patient’s stroke diagnosis.
For Stroke-3 quality measure compliance:
- Document a stroke diagnosis in the Hospital Problem List.
- Indicate current/history of A-fib/flutter as a Hospital Problem or Problem List Diagnosis.
- Ensures BPA triggers, alerting you that the patient doesn’t have an anti-coagulation discharge order.
To meet Stroke-2 quality measure compliance:
- Document the stroke diagnosis in the Hospital Problem Diagnosis.
- Ensures BPA triggers, alerting you that the patient doesn’t have an antithrombotic therapy discharge order.
Note: If neither step applies, document an Acknowledge Reason.
