Pyxis Issues Update

IT continues to work with the vendor to resolve the Pyxis connectivity issues. If you’re unable to access medications, try using another Pyxis machine or contact your local pharmacy.

Pyxis machines are on critical override. Here are some important reminders:

  • While using the Critical Override feature, utilize safety measures for pulling medications, including having the patient’s Epic MAR available.
  • If you can’t find your patient, use the Facility Search.
  • If the patient isn’t crossing over, create a Temporary Patient.
  • To remove a medication, remember to use the first three letters of the medicine to search for the name of the medication.

Blue screen recovery:

  • To recover a machine that has the blue screen below, notify pharmacy to complete a hard reboot by unplugging /plugging the machine or using the power switch on the back until the screen goes black.
    • Note: the blue screen may take a while to resolve.

Wasting controlled substances (CS) during downtime procedure:

  1. Please follow waste procedures at the Pyxis machine that you pulled the controlled substance from. It should transmit, even if it doesn’t say it on the screen.
  2. If you encounter a blue screen during the waste process, attempt to enter the waste activity on another Pyxis machine.
  3. If you were unable to complete a waste transaction on the alternate machine(s) (e.g., you get a blue screen during the process), then perform proper physical waste procedures at the Pyxis machine with a witness present. Place the waste into the CSRx container and document the waste via the downtime paper form for your entity. Details on the paper form need to be sent to your entity’s pharmacy department or Controlled Substance Diversion Prevention Team by the end of the shift. Details for documentation include:
  • Date/Time of Waste
  • Unit Area Wasted
  • Patient Name, MRN or CSN number
  • Medication Name/Strength
  • Amount wasted
  • RN Name
  • Witness Name
