Starting Monday, Oct. 16, every inpatient and observation patient must have Social Determinants of Health (SDOH) assessment documentation completed within 24 hours of admission. To support this, the SDOH section is included in the Assessments section of the Admission navigator.
These SDOH assessments are essential for identifying potential, underlying causes of health concerns or barriers to proper health maintenance and recovery. Completion of the assessment is the responsibility of the patient’s nurse on admission, unless otherwise indicated for each unit. Responses identified as high-risk will trigger a social work consult.
