New Social Determinants of Health Questions Added to eCheck-In

New social determinants of health (SDOH) questions were recently added to theNew social determinants of health (SDOH) questions were recently added to theeCheck-In process for physician office visits. Patients are prompted to answer new questions focused on food insecurity, transportation needs, utilities, living situation and personal safety. Questions help our clinical teams address fair access to health care for all our patients.Clinicians can view SDOH responses through the SDOH Wheel in Epic, available through Chart Review or Storyboard. The current 12 domains for SDOH screening questions include:

  • Alcohol use
  • Depression
  • Food insecurity
  • Living situation
  • Personal safety
  • Transportation needs
  • Medication affordability
  • Physical activity
  • Stress
  • Social connections
  • Tobacco use
  • Transportation needs
  • Utilities
