New Initiative Includes SDOH Access via Inpatient Storyboards Coming Soon
HM is launching a multi-phased initiative around social determinants of health impacting our patient population. As part of this initiative, we’ll make it easier for you to access the Social Determinants of Health (SDOH) Wheel in Epic.
The SDOH Wheel helps you identify and document patients’ social, behavioral and economic factors impacting them outside the health care setting. It includes 12 domains with screening questions, designed to uncover factors like housing status, food insecurities, social support and more.
The SDOH Wheel link was added to ambulatory Storyboards last year and soon you’ll be able to access this link on all inpatient clinical Storyboards. This helps you easily identify, assess and address your patients’ SDOH and screen them for any health-related social needs. Here’s how:
