Easier CareSense Pathway Enrollment for Heart Failure Patients

Heart Failure CareSense Pathways at HMH will have an easier enrollment process. Instead of manually enrolling these patients in CareSense, you can do it with one click. This will make the process easier and increase enrollment. Note: This option will only present for discharging patients, identified as meeting heart failure criteria. The final decision to enroll a patient is at the provider’s discretion.
When discharging a heart failure patient in Epic:

Click Yes under the Add patient to Communication/Education Pathway for Heart Failure?

If the patient isn’t discharging home, click No.
