Nursing Flowsheets Updated for Greater Efficiency
Complete the Charting by Exception Training in LMS

With the March 6 Epic Update, more than 200 flowsheet rows were updated in the Complex Assessment and Cares/Safety flowsheets. Changes included adding and clarifying some entry options and removing redundancies and infrequently used selections.

What You Need to Do
With these changes, greater emphasis is being placed on charting by exception. Please complete the Charting by Exception training in LMS (course # 1609048) as soon as possible to understand when and how it should be used.

Charting by Exception Overview
Charting by exception allows you to:

  • Spend less time on paperwork and charting.

  • Reduce variations in documentation quality and quantity.

  • Eliminate confusing and redundant charting.

When assessments are within range, you can enter WDL (within defined limits) and not expand flowsheet groups for unnecessary and time-consuming data entry. Additional charting is only necessary when results and assessments are outside of expected normal ranges.

Note: To focus on documenting exceptions only, options for expected normal ranges for a system assessment were removed for cascading rows, but can be easily referenced in the row information.
